How to apply:

Complete the HR 32 Salary Packaging form (download here) and email to memberships-sport@unimelb.edu.au

*Paper forms are not accepted.

Suspension and cancellation:

This membership type cannot be suspended, instead a cease packaging request must be made. You will then need to request to restart when you are ready to return.

Email a completed HR 32 form via email to memberships-sport@unimelb.edu.au, ensuring you have selected the cease packaging option.

*Please allow sufficient notice. Depending on the timing of your request, up to 1–2 additional deductions may occur.
